Attached is the PDH questionnaire that you must download, complete, and return to receive your PDH certificate.

You MUST be logged into the REMO program for the entire 2 hours of the presentation. Your log in and log out times are recorded by the program. PDH certificates will be issued only to individuals who return the completed questionnaire and who were logged in for the entire presentation.
